You're supposed to be able to regain those two ranks by doing the two of those three remaining quests for Burz gro-Karsh and Azzan. Turning it in to Burz gro-Kash, which you can do, delays that demotion, but the demotion NEEDS to happen for the Fighters Guild story-line to move forward. Turning in the quest at Anvil is supposed to bump you down two ranks. The first is that when you take on "Trolls of Forsaken Mine", you're supposed to be at the third-highest rank,with one last quest for Azzan and two last quests for Burz gro-Karsh. Yeah, I think I've figured out your problem, and it seems to be a combination of two things. Remember to only speak to him during the day in his office. See if experimenting with these makes Azzan's conversation options change and Advancement is now highlighted. See if using the console to demote yourself to Protector or Defender then makes Azzan have an option to progress in the guild. From what I've read on UESP Azzan only gives you the option to advance and then offer a new quest when you are at the rank of Protector or Defender but as you're now a Warder you have progressed past those ranks.
Normally you could advance the quest to the next stage which usually fixes any glitch preventing you from progressing and it has a small effect but with no active Fighters Guild quest that's not an option. I'm a bit stumped on what console command will work and have the least impact.
